XYZ Corp. by [Your Name, Organization, Address, Phone Number,
Fax Number, Email Address]." ***
You can opt to have your logo appear on the packaging as well

Once you've submitted your information, we'll
send you a free sample Rx CD "Handling Rejection" and
some materials that will orient you to the Rx CD System.
This CD will give you valuable insight into what
you can expect to be on each CD in the Attribute, Behavior and
Values Rx CD Library. Your free sample package will also include
a complete catalog of all available Rx titles.
